You are asked to research teamwork, communication and health assessment and answer the short answer question below.
Your response needs to Include an introduction (100-150 words) before your response to the 3 questions below and followed by a conclusion (100-150 words). The use of short headings (no more than 3 words) is advised.
You will need to use peer reviewed journal papers to support your discussion for question 2 & 3. Question 1 does not require referencing.
Stick to the word count. Reference list is not included in the word count, however in-text referencing is. Make sure you include a reference list (containing only the references used in-text only) at the end of your document. The reference list should always start on a new page.

Questions:
Using ISBAR format, document a handover from ED to medical ward were Frank will be admitted. Keep your handover concise, do not include all the details from the case study. You need to highlight only the relevant clinical information that are pertinent to Frank in this ISBAR handover. (250 words)
Discuss person centered care and how it applies to nursing practice. This question should have in-text citation. and should include peer reviewed journals no more than 5 years old. (450-500 words)
Discuss interdisciplinary teamwork and how it applies to Frank’s case study. In your response identify at least 2 other member of the interdisciplinary team that should be involved in Franks care and why. (450-500 words)

Case study
Mr Frank Skinner, a 58-year Aboriginal man who lives in a small regional town. He works at the local primary school as a teacher and is well-known and respected elder within the community.
Frank has lived all his life in the local town and has a large extended family. He currently lives with his wife, his daughter and son in-law, three grandchildren and one great grandson. They live in a four-bedroom home and have regular visitors coming and going from the home. It is not unusual for his wife Mary senior to cook dinner for 12. Mr Skinner has four children whilst Mary junior and her family live with him, one of the other children live within the town, one is interstate and his third child, Barry died in a motor vehicle accident at 15 over 35 years ago.Frank enjoys going to the local rugby league games in winter and catching up with his extended family and mates as well as other community and cultural events. As an elder, he is expected to participate and attend many community events. At times, this affects his work who are supportive of Frank due to his long term standing in the community.
Frank smokes 10 cigarettes a day since the age of 15 years; he has not drunk alcohol since the loss of his son Barry 35 years ago. Frank walks to work and home every day about 20 minutes, leaving the family car at home for the family to utilise during the day. His wife Mary senior looks after the family home and does all the cooking usually consisting of meat and vegetables. At time, she struggles to cook for so many and fain it difficult to manages the family budget with so many grandchildren to assist and help.
Frank Skinner Presents to the emergency department
It is about 1300hrs when Frank is brought into the Emergency Department by his wife Mary. Frank sits down and his wife who seems to be panicked comes over to the Triage desk and starts to tell you about her husband Frank. She starts by telling you he was just walking in the door at home as he came home early from work as he was not feeling that great, he had a headache. He sat down and complained of his headache, He sounded tired. He went to get up and had difficulty getting up as he did not have enough strength to get up unassisted. This is why he was brought to the Emergency Department, as this is not normal for Frank, he is stubbornly independent.
This was when you notice Frank is sitting in the waiting room with his head resting into his right hand and looks to be leaning over to one side.
You continue to ask Mary about Frank, she tells you he is having trouble speaking and that she had difficulty getting him into the car as he was leaning to the right side all the time and was very unsteady on his feet as she was getting him into the car.\You ask (wife) to bring Frank over to the triage desk so you could ask Frank some questions, you notice the difficulty (wife) has in getting Frank to stand and decide it is safer for Frank to be put into a wheel chair. You go and get a wheel chair and as you assist Frank into the chair you notice he is leaning to the right, he has a marked right sided facial droop and has limited control over his right arm.
