This assignment will provide you with the opportunity to demonstrate your understanding of the Recreation Therapy Process through the creation of an individual intervention plan. 

Instructions

Using the case study on the following page, create an individualized intervention plan for the person in the case study. Be sure to include the following components:

• Person’s Name
• Why Admitted
• Subjective and Objective Data
• Concluding Assessment Statement
• Needs and Strengths 
• Goals and Objectives
• Goals: Identify four (4) goals for recreation therapy.
• Behavioural Objectives:  Create 2 behavioural objectives for each identified goal. 
• Intervention Plan: Identify 2 recreation program interventions. In an appendix, include a “mock” program description for each intervention with the following information:
• Program name
• Program description
• Benefits of the program that would address the needs of the person. Include at least 3 references for each program that demonstrates how program is evidence-based (use APA referencing).
• Evaluation Plan: Describe how you will monitor the person’s progress and know if it is successful.

CASE STUDY

Paulette Brown is a 32-year old woman who is from Kitchener. She has been admitted to a short-term psychiatric unit for severe depression and anxiety. She will likely be at the hospital for at least one month. Paulette’s assessment identified that in addition to depression she was facing significant medical issues.   You are the recreation therapist assigned to her case and will be working with an interdisciplinary healthcare team to support Paulette.

In the last six months Paulette has experienced two major events: 

1. the death of her mother from kidney cancer, which doctors related to obesity; and 
2. being newly diagnosed with a pre-diabetic condition, which necessitates that she loose weight.

Paulette also has a low level of physical fitness. Her history revealed that she has had weight problems since childhood and has been unsuccessful in dieting. The consequences of her ongoing problems with weight have led Paulette to a sedentary and isolated lifestyle. Paulette revealed that because she was embarrassed by her weight she has withdrawn from social activities. She states that she is lonely and has no friends. She says she wouldn’t know what she would do without her sister who lives 3 hours away but she talks to on the phone regularly. She also demonstrates low self-esteem. Her major leisure activities include watching television and playing computer games. She likes to watch shows related to cooking, home decorating and animals (she has a dog and a cat). Her diet consists of fast foods that can be easily delivered to her house. Paulette is aware of the seriousness of her health problems and she is able to understand what needs to be done to improve her health.Prior to her mother’s death, Paulette planned and prepared the family meals and coordinated her mother’s care. The stress of her mother’s care added to her ongoing weight problems and increased social isolation. Paulette has expressed her desire to change her behaviours in order to develop a healthier lifestyle. Her community environment, including public and private recreational agencies, does offer opportunities for participation.
