Your response to the case study information 

You are a student on placement at the ABC Community Centre and have just been allocated a new client who has been referred by Community Health for support after a falling out with some members of her family over the way she was managing her health. You have met the client and completed the above assessment tools. 

You have now been asked to present the case during a case consultation session with the staff team. The team is a mix of professionals i.e. Social work, Human service workers, Psychology, Occupational Therapy and Nursing so your notes must provide enough information to ensure they understand the client, the context and the social work/ human service perspective. 

To prepare you need to apply knowledge from this course to the case study to write an empowering overview of the assessment phase of the helping processes. 

The following points will help to guide your thinking. 

1. Give a brief dot point introduction to the client, date of first visit to the agency.

2. Please use your knowledge of the strengths perspective (300 – 400 words), ecosystems theory (300- 400), and the concept of intersectionality (300- 400) to interpret the tools as an empowering assessment of the current situation. 

3. Then from the assessment, identify one problem (20 words) to focus on, write a needs statement (20 words). You must be able to justify this need by referring back to and citing theory (300). (The need is what has become apparent from the assessment you have just completed.)

4. Describe how professional values and ethics have influenced assessment with this client and how you will demonstrate an empowering approach to practice- support your claims by referencing specific parts of your professional code of ethics. (400)
