They are the individuals who are called on to ensure the organisation (and staff) have met legal and ethical obligations. It is the managers who complete the critical incident reports, conduct structured debriefing sessions and recommend further staff training so the incident is avoided altogether or dealt with more appropriately.
 

2. Brief
In this assessment a critical incident has occurred involving a client and one of the Brighter Futures staff members. As a result, you are to plan and conduct a debriefing session with the staff member directly involved. You will need to establish that the staff member acted in accordance with their legal and ethical obligations and record the incident officially on the approved critical incident form. Once this is complete, you then need to ensure the staff member’s well-
being by instigating a staff support plan, using the approved form. As a co-worker was indirectly involved in the critical incident, you will need to interview them as well and instigate a staff support plan if needed.

3. Questions/Tasks

Scenario

A critical incident has occurred at CareShore in the Brighter Future program.  Samantha (Sam) the community services program worker has been working with the White family. The parents (Jo and Doug) have had a history of alcohol and drug usage but had been managing well. Sam had been doing regular home visits, working with Jo especially, and ensuring that their daughter Milly was going to preschool. Sam had called in to see Jo and talk about Milly and how she was going in preschool. Although no one answered the door Sam could see a car in the driveway and figured that someone was home. Then she heard Milly crying. Sam rang CareShore for you, but as you were unavailable spoke with Kie on reception. Kie was not really sure what to do but felt Sam should not enter on her own.

Sam was getting increasingly concerned as she could now hear Milly crying for her mum (Jo) loudly. Kie and Sam agreed that Sam should try to go inside. Kie wasn’t able to contact you either, so she rang the Police.

Sam tried to open the door and found it was unlocked, so went inside. Sam found Jo on the kitchen floor, unresponsive, with some gashes to her body. There was lots of mess, blood, some broken glass and Milly wandering around very upset (crying). Sam comforted Milly and called Kie to report what she had found. Kie called an ambulance for Jo and let Sam know the police were coming. Kie finally contacts you and explains the situation and that Sam wants to know what to do next.

A few days later, Sam is back in the CareShore offices and more information has become available surrounding the incident. Apparently this was a case of domestic violence stemming from a violent argument between Jo and Doug. 

Sam is still visibly quite upset at the event. You are scheduled to meet with Sam and facilitate a structured debriefing session. 

  1. a. When should the debriefing session be scheduled and where should this occur? Name 2 factors to take into consideration for both the timing and a location. 
  2. It needs to be established that Sam has fulfilled her legal requirements surrounding the alleged violent incident at the White household. Before debriefing Sam and running through the critical incident form, fill in the table below to compare Sam’s actions and responses to the expected responses.
  3. An incident report is required for the organisation. 
  4. Complete 32509_03 CareShore Accident, Incident, Critical Incident form (provided in Appendix 1 at the end of this document) for Samantha using the information presented in the scenario.  Tip: Have a friend read the scenario and act the role of Sam (Samantha). Ask the debriefing questions created in Q1, verify Sam’s actions in Q2 and work through the critical incident form capturing Sam’s responses onto the CareShore Accident, Incident, Critical Incident form.
  5. Staff support plans are required to be completed (one for Sam and one for Kie).
