On my practice placement at renal unit, I met a new haemodialysis patient, referred to as Mrs W due to protect confidentiality in line with code of conduct (NMC, 2018). I introduced myself to Mrs W as a student nurse that will be looking after her alongside my supervisor. As I got to know her and chatted to her about her dialysis journey, how it all started and the impact on her family. Mrs W started feeling unwell from November 2020 when she received the devastated news of her daughter being diagnosed with ovarian cancer. She became worrying about her daughter, and what she could have done to help the daughter which made her very anxious.

This reflects to how receiving sad news can have impact on someone health. After the sudden death of her daughter in January 2021, Mrs W started having abdominal pain, she could not eat nor drink and started experience same symptoms and feelings like what her daughter had. She was dehydrated which made her feel is the reason she could not pass urine and could not concentrate to anything concerning her wellbeing apart from the sorrow and guilt she had inside. Mrs W had appointment with her doctor, and after the test the doctor sent Mrs W straight to A n E hospital where they did blood test and the result were shocking as her creatinine level were extremely high (abnormal) 1200. The hospital diagnosed Mrs W with kidney failure and performed emergency operation to put line on her neck for dialysis as her kidney was 0% functioning. Due to the loss of her daughter Mrs W did not realised that she was not passing urine, as she had been neglected herself with her diet and personal care, she was depressed, confused and frustrated with the situation she was in. As she is accepting her situation for being on dialysis and the help of family which she has a son and daughter who are doctors, Mrs W has improved on her health emotionally, physically and psychologically.

Her diet has improved as she can prepare healthy meal which does not impact on her dialysis. After talking to Mrs W and heard her story, made me determined to pay close attention in my future nursing when I talk to patients or family member especially after the loss of the loved one, that I might be able to figure out some of the problem Mrs W mentioned by giving advice and encourage the patient or family members to seek medical help. I joined dietician clinic nurse when she was assessing Mrs W diet and I was able to read and interpret her blood result, the nurse was pleased with Mrs W diet as the blood result shows everything was on normal level especially the calcium and phosphorous and encourage Mrs W to continue with the same diet and reassured her if she needs help, she can contact her anytime. Mrs W has previously worked as a nurse this benefit her to control and maintain her diet, she is cautious of what she can eats. During the time I spent with the dietician nurse I learn how the higher level of phosphorus can lower levels of calcium in the blood, and this cause more calcium to be pulled from the bones to raise the blood calcium levels but this weaken the bones have impact on dialysis patient
