This is where you present your actual and potential diagnosis. Remember these are based on the data that you have gathered. In this section you need to structure both actual and potential nursing diagnosis using the North American Nursing Diagnosis Association’s Taxonomy of Nursing Diagnosis (NANDA). We discussed and developed these within each lecture throughout the semester (first year). There is a link to the Nursing Diagnosis textbook available on the LMS in the online resources tab. Remember an actual diagnosis reflects immediate and currently present signs and symptoms, while the potential diagnosis is based on risk factors that might develop into an actual problem for the patient.

This is where you present information about the interventions that you have implemented to address each nursing diagnosis (both the actual and potential). You can present these again with sub heading and address each individually. Remember you must include one (1) independent and one (1) collaborative nursing intervention to resolve the patient’s actual problem and one (1) independent (OR collaborative for NURS90053) nursing action to avert the patient’s potential problem is to be discussed.
In this section you are encouraged to link back to the literature. Consider best practice guidelines.
Remember dependent nursing actions are those requiring a medical order are not to be discussed for NURS90130 e.g., administration of medications, IV fluids etc (This is suitable for NURS90053)
Evidenced-based rationales with reference to the current literature are to be provided for each nursing intervention.
In this section students need describe the expected patient’s subjective and objective data after nursing actions have been implemented and outline the expected observable patient response to nursing care in line with the established objectives
When writing outcomes, the nurse should ensure that the outcome statement is written in measurable behavioural terms.
RUMBA is useful to use hereRealistic, Unambiguous, Measurable, Behavioural and Achievable.
The outcome statement should be written sequentially, and with timeframes.
