Tertiary Prevention and Chronicity:
Your task is to develop and present a 10 – 12 minute evidenced based power point presentation, as a recorded video. The power point presentation will allow you to demonstrate your understanding of factors that: underpin the challenges people experience when living with a chronic condition; the determinants of health that may influence the person’s health and the health Tertiary Prevention and Chronicity:literacy skills necessary for the person to be able to receive best health care.

SCENARIOS.
Choose one of these chronic condition scenarios for your presentation. All people in the scenarios live in outer city Brisbane with a travel time of 20 minutes to the closest major tertiary hospital.
• 85-year-old woman, Joan with osteo arthritis and living alone (mobility and self-care impacted from chronic pain and the deformity of joints in her hands and feet)
• 12-year-old boy, Oscar post osteosarcoma treatment 18 months ago.
• 45-year-old man, Paul post ischemic stroke (CVA) 3 months ago (on return to work program 3 days per week). Mild right sided hemiparesis, with mobility impacted. Mild aphasia, affecting speech only.

Slides should include and address:
• Title page. Introduce yourself, the chronic condition and its pathophysiology, and briefly state why you chose this case.
• Outline the modifiable and non-modifiable risk and protective factors associated with the chosen chronic condition.
• From a disability perspective, identify which of the determinants of health need to be considered for the person in the scenario at this time and why.
• Describe 1 health promotion tertiary prevention strategy that would be relevant for the person in the scenario. Explain why.
• Describe 2 health literacy skills that would be a priority to assist the person in the scenario. Explain why they would be a priority for the person.
• From a perspective of Health Promotion (ie. control of one’s health and wellbeing), describe what you would look for if the person in the scenario’s health literacy skills were effective.
• Conclusive slide. Summarise the key points from the presentation and identify 1 important point that you learnt from this task that you will be more mindful of in practice. Explain why.
