Mrs Smith is a 48 year old widowed mother of Max, almost 18 years of age. She lives in an Aged Care Facility following a motor vehicle accident where she sustained an acquired brain injury. Her general health has been stable; however she recently developed a chest infection, requiring treatment.The general practitioner (GP) ordered oral penicillin to be administered four times a day. The first dose was administered on Monday evening and Nurse Florence administered the second dose at 0600 on Tuesday and left Mrs Smith in the care of Janice the Personal Care Attendant (PCA) to attend her hygiene needs.Following the second dose Mrs Smith became severely dyspnoeic, her face became flushed and she was appeared distressed. Janice noticed her distress and called for Nurse Florence’s assistance. Unfortunately, Nurse Florence was attending another resident who had collapsed in the shower and was unable to come immediately.As Mrs Smith became increasingly distressed, she attempted to sit on the side of the bed to breathe more easily. She overbalanced and fell to the floor, hitting her head on the bedside table. Nurse Florence arrived some minutes later whereupon she and PCA Janice manoeuvred Mrs Smith back to bed with the aid of a lifting machine. She had a deep laceration to her temple which was bleeding. Nurse Florence called the GP, who informed her that he would attend on his way to the clinic. In the meantime he ordered that a dressing be applied to the wound and to “keep an eye” on Mrs Smith for any changes in her condition.Nurse Florence then called Max and explained the fall and the injury, but stressed that Mrs Smith seemed okay. Max informed Nurse Florence that he was his mother’s medical treatment decision maker and that he was on his way to the facility.Nurse Florence asked PCA Janice to take a set of vital signs while she applied a temporary dressing. The bleeding had subsided and Mrs Smith seemed to settle. PCA Janice undertook a set of vital signs and noted on the chart that Mrs Smith’s respirations were slow and shallow but thought that it was probably as a result of the fall so did not tell Nurse Florence.The GP finally arrived and immediately ordered an ambulance. He asked what had happened and was told that Mrs Smith had been given the penicillin as ordered and then later became a little distressed and fell out of bed, but seemed all right at the time. By this stage Mrs Smith’s son had arrived and, after hearing what had happened, shouted: “You idiots, Mum is allergic to penicillin and you should not have given it to her”.The ambulance arrived and Mrs Smith was taken to the nearby hospital where she had a CT scan. She had sustained a cerebral bleed which would require immediate surgery.Nurse Florence was dismayed that she did not know that Mrs Smith was allergic to penicillin so she checked Mrs Smith’s admission papers and found that there was no record of Mrs Smith’s allergy to penicillin documented. She went home at the end of her shift and posted to her Facebook page that she was upset and angry that she had a patient with an allergy that was not documented and was now fearful of losing her job over something that was not her fault. She critically commented on the lack of respect her colleagues gave to the residents in the aged care facility and the “stupid” GP with whom she had to work.As for Mrs Smith, after surgery, she was transferred to intensive care where she remained in a coma but died a short time later.
