For unit of competency:
Assessments carry two potential outcomes for each unit of competency level, i.e. C (Competent) and NYC (Not Yet Competent). The latter outcome requires you to undergo more training and experience or provide further evidence. On completion of the unit, if the assessor is satisfied that you have provided appropriate evidence and
satisfied all requirements necessary to meet criteria in line with the Rules of Evidence, you will be awarded C (Competent). If it is considered that you failed to meet the necessary requirements, you will be awarded NYC (Not Yet Competent) and may be reassessed in accordance with the policies and procedures of EQUALS. To be assessed as competent for this unit, you must demonstrate a satisfactory result in all assessment activities. Final assessment results are to be recorded on the Summary of Assessment form.

Tasks carry two assessment outcomes, i.e. S (Satisfactory) and NYS (Not Yet Satisfactory). On the individual cover sheet for assessment tasks, the assessor must mark S (Satisfactory) if you have satisfactorily completed the task, submitted all necessary evidence and met the assessment criteria. If you have not satisfactorily completed the task, the evidence is insufficient and all requirements have not been met you will be marked NYS (Not Yet Satisfactory).

Mr James and his family are concerned about his current physical health status especially in relation to oral health. The registered nurse asked you to provide a health teaching to Mr James and the family member about common oral health issues that occur as part of the ageing process. The registered nurse informed you that the person has hearing impairment and used hearing aids. Your health teaching must also include appropriate oral care in the elderly. As you walk into the assessment room, you see Mr James sitting on a chair with his eyes closed and his spectacles and hearing aids on the table in front of him. Use appropriate health terminology and communication strategies when providing the health teaching. This assessment could be undertaken in the simulated nursing environment, classroom or a meeting room in the EQUALS’ campus. Your assessor will assist you in setting the simulated environment. Based on the health information you provided to Mr James and his family member, you are asked to complete a progress note. This progress note will be used to communicate key information you provided to Mr James and his family to other colleagues during the handover. You could access the progress notes’ template provided with the checklist for this assessment task following the demonstration of appropriate actions related to this task. You need to complete the progress notes under supervision of your assessor.
Use appropriate health terminology when completing the progress notes complying with the principles underpinning nursing documentation and policies of the healthcare setting. A summary of your workplace policy on documentation is provided at the end of this task. You are required to request your classmates to play the roles of Mr James and the family member. You must be prepared to deliver the health teaching session. You may draft a possible conversation between a nurse and the person engaged in the role play prior to the assessment session.
