25-year-old female, Jessica Brown, presenting with withdrawal management from Ice, 2-3 points IV use, for the last 10 years. Has used cannabis 0.5gram per since age of 15 to 20, smokes cigarette 20-25 per day. Nil known medical issues. History of anxiety first diagnosed at teenage years and depression diagnosis 5 years ago. Planned 10-day admission with follow up in long term rehabilitation for 12 months in Windana. Jessica has been referred to our service by her mother, who also gave Jessica ultimatum that if she does not complete the detox program and the rehabilitation, she is not welcome home anymore and will loose all support including financial support. Jessica could potentially be homeless if she chooses to self-discharge. She has also recently lost her licence due to driving under influence of drug. 123

There are many studies which indicates that practice of Motivational interviewing (MI) leads to positive outcome on therapeutic alliance (Haugan, Berge, Breiseth, Sund, & H…, 2019).
MI can be defined as therapy that is client–centred counselling technique which is to resolve a client’s uncertainty towards behaviour change, thus turning their intrinsic motivation into action (Gill, Oster, & Lawn, 2020).This intervention was initially developed in the area of substance abuse (Gill, Oster, & Lawn, 2020). Motivational interviewing is done in collaborative style which strengthen the consumer’s commitment and motivation to change, the style of counselling is person centred which addresses issue of ambivalence, the collaborative sessions are goal directed with using language of change, exploring consumer’s own reason for change with the atmosphere of compassion and acceptance. (Dickerson, Moore, Rieckmann, Croy, & Venner, 2018)
There are 3 core skills of communication that is important with (MI),
1 informing: which is factual knowledge about treatment or diagnosis,
2 asking: with open ended questions to understand consumer’s perspective and problems,
3 active listenings: by giving a reflective statement to demonstrate our understanding. (Gill, Oster, & Lawn, 2020)
There are communication styles which is important part of (MI). Following style which means the practitioner is listening and following the conversation without interjecting their own agenda, this style enable client to have faith their own knowledge, the practitioner be with client and let the client work the problem out in their own way. Another style of communication is the guiding style of communication where the practitioner is good listener but also offers expertise when needed. The above-mentioned style is not single one better than the other, with different consumer different style might be more useful, therefore it is up to the art and expertise of practitioner to use the appropriate style of communication according to the consumer’s need and situation, being flexible to change approach when needed. (Gill, Oster, & Lawn, 2020) Motivational interviewing was found to decrease desire of substance use and increase self-efficacy among Iranian woman with substance abuse (Oveisi, Stein, Babaeepour, & Araban, 2020). The reason that (MI) is known to be effective among clients with substance misuse is because often client have ambivalence about change of habit and often not sure if they do want to engage in psychotherapy either.
