There are an increasing number of patients reaching end stage renal failure (ESRF) and requiring a form of dialysis (KHA 2016a and ANZDATA 2019). The majority of new patients to haemodialysis (HD) commence their treatment within the Tertiary Hospital. Currently the Tertiary Hospital is funded for 96 chronic patients to have their HD in-centre, but provides HD for 105 plus patients.

The increase of in-centre HD patient numbers has compromised HD nursing services to a point where the incidence of adverse events have risen, as per the Tertiary Hospital Risk/Incident report 2019. The most common of these are access-related complications; medication errors and dialysis prescription errors.

Australian Institute of Health and Welfare (AIHW 2019) shows a national average of hospital acquired complications (HAC) at 0.75 cases per 10,000 patient days, with the Tertiary Hospital showing at 1.29 cases per 10,000 patient days which is 58% more than the national average.

HD patients’ non-adherence in attending Renal Team clinic appointments, where physical assessment and review of medication and dialysis prescriptions are carried out, can lead to complications requiring hospital admission (Som et al. 2017 & White et al. 2002). This affects patient health outcomes and also increases cost burden on the hospital (Rocha & Pincho 2018). A medication error, such as omission of erythropoietin, can lead to low haemoglobin and result in the patient requiring a blood transfusion, costing $405 per transfusion (Australian Red Cross 2016); HD access-related infection means hospital admission for intravenous antibiotics at a cost of $5,500 per admission over a 5 day stay in hospital (AIHW 2019); and dialysis prescription error leading to acute pulmonary oedema will require hospital admission with a cost of acute dialysis $80,000 per year (KHA 2016b).

The culture to promote safety and quality care is heavily emphasised in hospitals and nursing currently (avies et al. 2016, Ulrich & Kear 2015). It is multi-dimensional and requires leadership to improve the culture of safety and quality (Davies et al. 2016, Ulrich & Kear 2015). The role of Clinical Nurse Consultant (CNC) has shown to promote

health care delivery, patient outcomes and encourage staff (Baldwin et al. 2013). A CNC will e

nsure HD patients receive safe and quality care as per The National Safety and Quality Health Service (NSQHS) Standards (ACSQHC 2011) and the Tertiary Hospital Nursing and Midwifery Strategic Plan 2015-2019.
