Tom is a 76 year old male who is admitted to the ward from the emergency department after experiencing left-sided chest pain radiating down his left arm for approximately 2 hours. He is now pain-free and a provisional diagnosis of angina has been made by the emergency doctor. You notice that he has oedema to his feet and ankles and shortness of breath on exertion, which Tom has only noticed over the last 2 days.

As part of Tom’s health assessment, you discover that he smokes 40 cigarettes/day and weighs 110kgs. His height is 175cm and he has a history of hypertension and congestive cardiac failure. His medications include: Metoprolol 25mgs twice daily for his blood pressure, and Paracetamol 1gram as required for aches and pains.
He lives with his wife and their daughter who is married with 2 children lives around the corner. His daughter visits them every day when she picks up the children from school.
