Mr. ralph is 75year old men living alone in two-bedroom house. He is having a history of multiple disease condition depression, type 2 diabetes mellitus, osteoarthritis which limit his mobility. He drink moderate alcohol and does smoking. His wife died one year ago. He has one son who lives with his family near to his home. He prepare his own meal, but most of the time he eats frozen meals, because cooking require lots of efforts. His BMI is 32 and obese. He just came back from hospital after having a fracture on his ankle due to fell down in the shower. He refered to community nurse for his next dressing on ankle
