Assignment details Part 1 Reverse Case Study (approx. 1000 words)
**Headings are allowed and are based on each step of the Clinical Reasoning Cycle as detailed below.
Introduction: –
• Brief details about the patient situation including sociodemographic information
• Briefly introduce the main nursing diagnosis or patient problem
• Briefly describe the key learning from this case
Collect cues/information
• Outline quantitative data and qualitative information you have about the patient. This information can be presented in a table or written briefly in essay style. This information can be collected during clinical placement from: handover, patient charts, diagnostic and laboratory results, any findings from medical and nursing assessments previously undertaken and your own observations and interactions with the patient.
• Quantitative data might include a table detailing information about any medications and fluids charted for the patient including drug/fluid name, drug class, dose, indication, administration instructions including relevant medication calculation formulas. Identify compatibility information with other medications and fluids if relevant (IV medications); vital signs, fluid balance, pathology results
• Qualitative information might include patient’s social history including details about family and how involved they are in the patient’s care; patient’s perceptions of hospitalisation, patient’s reactions and coping with medical and nursing interventions and treatment
Identify problems/issues-
• Based on the information that you have so far, identify the main patient problem or issue, and identify an accurate nursing problem and nursing diagnosis
• If your patient is a paediatric patient – consider the role family will play in supporting the child with this problem
Establish goals
• Specify what you want to happen and when, and the desired outcome by establishing SMART goals. “Goals must be SMART (Specific, Measurable, Achievable, Realistic and Timely) and designed to address the nursing diagnoses previously identified. Without SMART goals, the nurse cannot determine the efficacy of their actions” (Levett-Jones, 2018).
Take action
• Describe the interventions taken/not taken. Describe what happened with your patient.
• Were any actions taken by the patient and/or the patient’s family?
Evaluate outcomes
• Tell the story about what actions were taken or not taken, who was involved, and the outcome for the patient including those taken by the patient and/or the family
• Identify the points at which error/s occurred or could have presented as a risk to the patient if they had not been identified or interventions were not put in place
• What interventions could have been implemented to prevent the deterioration of the patient?
