Overview

During your work placement, you will be undertaking a variety of duties and tasks that relate to your course and the role of a community services worker or manager. You will need to successfully demonstrate that you can perform tasks required in the work environment and collect contract .

This portfolio will be broken down into tasks, and these need to be submitted in three parts following the training and assessment plan agreed upon with your Workplace Assessor (WPA). This is Portfolio 2 of 3.

Select three (3) clients with mental health issues and co-existing needs in your workplace, and in consultation with your workplace supervisor, complete three (3) client interviews, assessments or referencing meetings where you:

1. provide information to the client and /or their family
2. listen, discuss and analyse any issues
3. plan and prioritise actions to achieve negotiated goals
4. provide evidence of a strengths-based approach to assessment
5. identify duty of care and dignity of risk considerations
6. provide referral service options or discuss the effectiveness of existing services that the young person is engaged in.
