Introduction
Cerebrovascular accident (CVA) is one of the most common diagnostic categories encountered by occupational therapists practising in adult and older adult rehabilitation services. The frequency of clients referred should not give the impression that this condition is unchallenging. CVA frequently results in lingering complex disability where a number of client factors can impact on the ability to participate in a range of occupations. But CVA also constitutes a stable neurological lesion, which although posing numerous challenges, also offers much scope for rehabilitation and hope for the client and significant others.
In this assignment you are required to synthesise and paraphrase knowledge and evidence around the occupational therapy intervention with a client who has sustained a CVA. Use the following case study as a reference point and to frame your assignment, but bear in mind that the priorities are that you articulate the unique contribution of occupational therapy in the rehabilitation a client who sustained a CVA and how client factors may impact on occupational performance, and how these are targeted by occupational therapists in their intervention programme.
Case
Mr John D, 57 years, works in a high-level executive post in a private business firm. He is married and his spouse also works in a private company as a secretary with very flexible working hours. John has a long history of hypertension, is mildly overweight and works long hours with no time to exercise. He and his wife live in a spacious luxury flat with good access. They don’t have any children. Their interests include watching TV and taking one summer vacation abroad every year.
John was referred to occupational therapy after he sustained a dense stroke of the non-dominant hemisphere, with hemiplegia, perceptual disorders and behavioural disorders with emotional and judgement issues. John is frustrated and wants to return to his job as soon as possible. He is very cooperative and is ready to do ‘whatever it takes’ in order to return to his important role in the private firm. He is not concerned about or self-conscious of the outward manifestation of his disability. He is more worried about being able to drive to work, undertake his work roles and being able to perform PADLs independently.

Section One – Discussion of Occupational Therapy Intervention
The unique contribution of occupational therapy is to empower John to address his occupational performance concerns and priorities, by enabling him to resume his various roles through change, adaptation and compensation. This occurs as John’s abilities improve or he adapts strategies to cope with the challenges of residual deficits.
Selecting an occupation-based model of practice, describe how you can develop an occupational therapy intervention programme for John D with realistic, specific and practical targeted outcomes. Take into account areas of occupation, client factors and contextual factors including the physical, social and institutional aspects of the environment. Also take into consideration possible lifestyle adjustments that might be necessary for John and his wife. Your programme should include inpatient and outpatient care, as well as jobsite and job-related aspects. Please note that you need to discuss two client factors in detail in the next section and will have to organise section one in a way that avoids or minimises repetition. (50 marks).
