Case Study: Bowel Cancer Alan is a 39-year-old with Chron’s Disease which he has had since he was 13 years old. This has been treated conservatively and caused him no issues or concerns. He has a routine colonoscopy in 1997 which indicated that the disease remained inactive. In 2002 Alan contacted his GP with a ‘flare up’ of his Chron’s disease and was treated with azathioprine. Alan was well controlled for the next five years. In 2007 Alan became self-employed and decided he was too busy to take time off work for the annual colonoscopy screening and informed the hospital he did not want any further investigations. In 2008 Alan was admitted to hospital with a bowel obstruction and the histology showed a Chron’s mass. During this admission the nurses and doctors impress the need to keep appointments as it is clear the disease is becoming active, and that he is at high risk of developing cancer. Alan’s wife attending this meeting and the nurse ensured that they both understood the information given to them. Despite this, Alan refused to have surgery to remove the mass and continued to manage his symptoms with medication for the next five years without attending any colonoscopy appointments. In 2013 he was admitted to ED with increased pain, pyretic and difficulty opening his bowels and passing urine. He also reported tenesmus. He is diagnosed with bowel cancer.
