Scenario:

Glenda Marina manages several DVD rental outlets in the metropolitan area. Each outlet has a small core of permanent staff and several casual staff who come in as required. In general, the employees at the outlets are friendly, responsible and work well together. Glenda personally trains them in their duties and pays special attention to ensuring that they understand and apply the safety procedures she covers as part of the standard operating procedures. 

While safety isn’t a major issue, it nevertheless remains in the back of Glenda’s mind. She doesn’t want anyone getting hurt; apart from the personal consequences, she has her staffing levels finely tuned and really can’t afford people to be off work due to injuries. 

Glenda’s accident, near-miss and injury statistics are similar to other groups of stores in the chain. The company uses Pareto charts (bar charts) to benchmark LTIFR (Lost Time Injury Frequency Rate) figures for each group of stores. 

The main injury reported is back injury caused by incorrect lifting; this is most likely occur when boxes of newly released DVDs arrive and the staff off-load them into the back room before sorting and coding them and placing them on the shelves. The second biggest cause of injury is strain, which sometimes occurs when staff are stocking the highest and lowest shelves. Falls account for the third main category of injuries, usually a result of overbalancing when putting up display materials such as posters and stocking the high shelves. 

Glenda’s group of employees normally benchmark in the best 50% of stores in the chain and, as the figures are reasonably low anyway, she is content. 

That is, she was content until this quarter’s figures came out! They show a marked increase of falls in all but one of the stores she manages. The resulting lost time due to injuries also weakened her labour utilisation performance figures for the period. She feels her own personal efficiency has decreased, too, because dealing with injuries diverts her attention from other important matters. Clearly, she needs to manage this outbreak of accidents before it becomes an epidemic.
