You are assigned to Mr. Lock, a 46-year-old, diagnosed with Leukemia. He has been undergoing chemotherapy on your unit, and yesterday he received his second treatment. You have now been on shift for 6 hours. When you ask how he is feeling, Mr. Lock states, “I just feel such terrible nausea, and I cannot keep anything down. Everything I look at makes me feel sick to my stomach.” When you assess your client, you notice he has dry mucous membranes and decreased skin turgor. Mr. Lock is ordered a regular diet but refuses to eat any solids as he tells you that “just the smell of the food makes me nauseous”. Mr. Lock’s wife is at the bedside and states “I worry about him constantly. We are just exhausted from the diagnosis and the chemotherapy, and now with all this nausea and vomiting, it is even more stressful.”
